A physician assistant works under the supervision of a medical doctor, providing support and care to patients in a number of fields, such as cardiology, oncology, and orthopedics. In this capacitiy Dr. Chasin brings years of experience in evaluating and treating newborn patients through to geriatric-aged patients for urgent and nonurgent medical conditions. She has also been a Family Medicine Physician Assistant providing primary care and urgent care for adult and pediatric patients of all ages in a community health clinic setting, evaluating and managing everything from sprains, fractures, and dislocations to colds, the flu, and COVID-19 to cardiac and respiratory emergencies and everything in between.

At this time, all visits are by appointment only.

When calling to schedule an appointment, please have the following information ready:

  • Patient’s full name (including middle initial)
  • Date of birth
  • Current address and phone number
  • Current insurance information
  • Purpose of the appointment

It is important to let the Patient Service Representative know all the concerns you would like addressed during your appointment so we can schedule the appropriate amount of time to care for your needs.

Please call the front desk at (845) 800-5118 to reschedule the appointment.

If your schedule changes, please call us to cancel the appointment at least 24 hours prior to your appointment time. This will allow us to give another patient the opportunity to be seen.

If you miss an appointment, you will be marked as a no show and we reserve the right to charge a $25.00 no show fee.
